What to Expect During Your Visit
Our team of staff and doctors will work together to evaluate your eyes and provide relevant eye care treatment information, review your vision benefits and payment options, fit you for glasses or contact lenses, and focus on exceeding all your expectations. If at any time you have questions during your visit, we are here to answer them!

New Patients and Returning Patients
We are committed to meeting the needs of our patients. The feedback we receive indicates a high need for efficiency and safety in the office for both patients and staff.
To accomplish this, we request all required patients to complete the online Registration Forms offered in the link below prior to their visit. When you schedule your appointment, we will ask you for your email address and cell phone for this purpose. You will receive a link several days before your appointment via email or text for access to the forms. You will be able to complete them in the comfort of your own home and with easy access to information you may need to accurately complete the forms.

When you press on the Registration Forms button below you will see 2 forms. Please read the descriptions to complete the correct form.
Patient Registration Form
The NEW Patient Registration Form should be completed by:
- NEW patients who are visiting us for their first appointment at SkyVision
- Existing patients who have not been in to see us for 3 or more years.
This form focuses on basic demographic, insurance, and medical history information.
The RETURNING Patient Registration Form should be completed by:
- Any Returning Patient who has been here in the last 3 years.
This form is shorter in length and focuses on basic demographic information and insurance information.
If you have records from your other doctors that you would like for us to have here at SkyVision, please fill out the Release of Records Form and give to your doctor(s) or fax to us at 440-892-3416 for help in expediting your records from them.

SkyVision Centers believes in providing the best value for you and your family. We have products and pricing that aims to accommodate all budgets and our office participates in most insurance programs. For patients who have little or no vision insurance coverage, flexible payment programs may be arranged. To learn more about our payment options, please call our office directly.
Our office accepts cash, check, and major debit and credit cards including MasterCard and Visa, as well as Care Credit.
Since there are so many plans and coverage varies widely, please call us to find out if your plan covers care in our office. Our staff will verify and explain your benefits before your visit at no charge.
Medicaid coverage is only accepted as supplemental coverage.
We do not accept Medicaid as Primary Insurance coverage.

- Medical Savings Accounts (MSA)
- Cafeteria Plans for most of your eye care needs
All of the above plans can be used for all your eye care and eyewear needs, including eyeglasses, contact lenses, routine office visits and medical office visits.
The Care Credit card is available to help people pay for health care expenses for up to 12 months no interest financing. We accept Care Credit for all of our services. Click here to learn more or apply for the card.
In the event you need a medical procedure such as a cataract surgery or glaucoma laser procedure, you may be instructed to go to an out-patient surgery center. Depending on the procedure, we use several different locations. Your surgical coordinator will provide the information necessary to confirm your in network benefits for the desired location.
We always encourage you to contact your insurance plan providers directly to make sure we are a participating provider, or we welcome you to contact our office at 440-892-3931 for additional assistance. All insurance plans vary in the amount of coverage, so it is important that you always check with your insurance plan to get confirmation on what they will cover, and what is your responsibility.

Need to Order Contacts?
Our contact lenses are competitively priced and we offer the option of convenient home delivery. Once your account is set up, you can order your contacts through our website on the button “Order Contacts” below. You must be set up by our office first to be able to order online.
Your contact lens prescription must be current, within one year. For your very best pricing, please plan your appointments for new prescriptions carefully to avoid running out and needing a “rush” or “holdover” shipment.
